INSTRUCTIONS for Professional Learning Unit Credit

for

Georgia Mathematics Conference

STEP 1:  A Prior Approval Form (on the GCTM website) must be signed by your school superintendent or designee and brought to the conference.  It confirms that attendance at the GMC supports your school or system goals.

You cannot earn PLU credit at the conference without it.

STEP 2:  Immediately after registering at the conference, turn in the Prior Approval Form at the PLU Desk.

STEP 3:  In exchange for your Prior Approval Form, you will receive a PLU Attendance Confirmation Form.  On the form, for each session attended, you will list the session #, name of session, and length of the session (in hours).
 

STEP 4:  At the end of EACH session, ask the presenter to initial your Attendance Confirmation Form.  You must obtain the initials IMMEDIATELY before leaving the session.  You will not be able to return to the site or to the presenter to obtain this verification later. 

Example:

Session #101   Math & Manipulatives, What Fun!                   1 hour              Initials

You will need to attend a total of ten hours of sessions to complete step 4.

STEP 5:  BEFORE leaving the conference, turn in the completed PLU Attendance Confirmation Form and a completed Conference Evaluation Form to the PLU Desk in the registration area.

 

STEP 6:  PICK UP a copy of the PLU Course Completion form that you will complete in your school or district.

STEP 7:  Return to your school or workplace and begin to implement some of the strategies or ideas that you learned at the conference.   You must do both of the following:

1.      Schedule a “sharing” session to share strategies and ideas that you learned at the conference.  This session should be appropriate to your responsibilities in your workplace.

2.      Schedule a “classroom observation” or “model teaching session” that demonstrates strategies or ideas learned at the conference.  This observation must be conducted by someone authorized by your school/system to make this observation and he/she must verify its quality.  This person must sign and date the PLU Course Completion form.

STEP 8: Following the completion of the sharing session and the classroom observation, the participant is responsible for returning the signed completion form (signed by the system-designated person i.e., Principal, Supervisor, etc.) to the Georgia Council of Teachers of Mathematics (GCTM).

The PLU Course Completion Form should be completed, signed and returned by Dec. 31, 2011 to:

STEP 9:  Completed verification of your PLU credit will be returned to you.  It is your responsibility to send this PLU credit form to your accrediting agency when needed.

GCTM will match and verify that all activities and artifacts (prior approval form, attendance confirmation form, and completion forms) are received and in order.  GCTM will return a certified course completion form to the participant. 

It is the participant’s responsibility to submit the final documentation to the certifying agency.
